A numeric value, assigned to each allele and summed across a diplotype, used mainly for CYP2D6 to translate genotype into a predicted metabolizer phenotype.

In depth

Each allele is assigned a value reflecting its functional status — no function scores 0, decreased function an intermediate value, normal function 1, and an increased-function or duplicated allele more than 1 — and the two allele values are added to give the diplotype's activity score. CPIC maps ranges of this score onto the standard phenotype categories, with 0 corresponding to a poor metabolizer and progressively higher scores to intermediate, normal, and ultrarapid metabolizer.

The activity score system allows a continuous, function-based estimate rather than treating every diplotype combination as a separate category, and CPIC periodically updates the score-to-phenotype boundaries as new allele function data accumulate.

Clinical pearl

Because CPIC periodically revises which activity-score ranges map to which phenotype, a genotype reported some years ago may be re-classified into a different phenotype without any new testing.
