A combined effect of two drugs acting on the same endpoint that equals the effect predicted from their individual dose–response relationships, as if one drug were partly substituted for the other.

In depth

Additivity is the null reference against which synergy and antagonism are judged. Under Loewe additivity, the combination doses (a, b) that produce a given effect lie on a straight line on an isobologram joining the equi-effective doses of each drug alone (A, B); for drugs with non-linear dose–response curves, additivity is not the same as arithmetically adding the two effects.

Additive effects are typical of drugs with the same mechanism or converging physiological actions, such as two antihypertensives lowering blood pressure or two anticholinergic drugs adding to the overall anticholinergic burden.

Clinical pearl

Cumulative anticholinergic or sedative burden from several individually low-risk drugs is a common, preventable cause of falls, confusion and constipation in older adults.

Aynı yanıt üzerinde etkili iki ilacın birlikte oluşturduğu etkinin, tek başına doz–yanıt ilişkilerinden beklenen etkiye eşit olmasıdır; bu durumda ilaçlardan biri diğerinin yerine kısmen konmuş gibi davranır.

Aditivite, sinerji ve antagonizmanın değerlendirildiği sıfır (referans) durumdur. Aditif etkiler aynı mekanizmaya veya birbirine yakınsayan fizyolojik etkilere sahip ilaçlarda tipiktir; iki antihipertansifin kan basıncını düşürmesi ya da iki antikolinerjik ilacın toplam antikolinerjik yükü artırması buna örnektir.

Tek tek düşük riskli birkaç ilacın oluşturduğu toplam antikolinerjik veya sedatif yük, yaşlılarda düşme, konfüzyon ve kabızlığın sık görülen ve önlenebilir bir nedenidir.
