A graph relating the dose or concentration of a drug to the magnitude of its effect (graded curve) or to the proportion of a population showing a specified all-or-none response (quantal curve).

Graded curves plotted against log dose or concentration are typically sigmoid and are described by the Hill equation relating effect (E) to concentration (C), which yields efficacy (Emax), potency (EC50) and the Hill coefficient (n), a measure of steepness; the central portion is roughly linear. Quantal curves are cumulative frequency plots of an all-or-none response in a population and give the ED50, TD50 and LD50 used to estimate the therapeutic index.

Curve shifts are informative: a parallel rightward shift indicates reduced potency, as in competitive antagonism, whereas a depressed maximum indicates reduced efficacy, as in non-competitive antagonism. Some responses are non-monotonic (bell- or U-shaped).

Clinical pearl

Where the curve is steep, small dose increases can move a patient from subtherapeutic to toxic effect, so titration should be gradual and monitored; on the plateau, further increases add toxicity rather than benefit.

Doz–Yanıt Eğrisi

Bir ilacın dozu veya konsantrasyonu ile etkisinin büyüklüğü (derecelendirilmiş eğri) ya da belirli bir hep ya hiç yanıtını gösteren popülasyon oranı (kuantal eğri) arasındaki ilişkiyi gösteren grafiktir.

Logaritmik doz veya konsantrasyona karşı çizilen derecelendirilmiş eğriler tipik olarak sigmoid biçimlidir ve etkiyi (E) konsantrasyona (C) bağlayan Hill denklemiyle tanımlanır; bu denklem etkinliği (Emax), potensi (EC50) ve eğrinin dikliğini gösteren Hill katsayısını (n) verir. Eğrinin orta bölümü yaklaşık olarak doğrusaldır. Kuantal eğriler ise bir popülasyonda hep ya hiç tipindeki bir yanıtın kümülatif sıklık grafikleridir ve terapötik indeksin hesaplanmasında kullanılan ED50, TD50 ve LD50 değerlerini verir.

Eğrideki kaymalar yol göstericidir: sağa paralel kayma yarışmalı antagonizmada olduğu gibi potensin, maksimumun düşmesi ise yarışmasız antagonizmada olduğu gibi etkinliğin azaldığını gösterir. Bazı yanıtlar monoton değildir (çan veya U biçimli).

Eğrinin dik olduğu bölgede küçük doz artışları hastayı subterapötik düzeyden toksik etkiye taşıyabilir; bu nedenle titrasyon kademeli ve izlem altında yapılmalıdır. Plato bölgesinde ise doz artışı yarardan çok toksisite ekler.
