Median Effective Dose ED50

The dose of a drug that produces a specified effect in 50% of a population (quantal definition) or, in graded dose–response studies, the dose producing 50% of the maximal effect.

In depth

The ED50 is obtained from a quantal dose–response curve, a cumulative plot of the percentage of subjects showing an all-or-none response (e.g. analgesia, sleep, seizure protection) against log dose, usually analysed by probit or logistic regression. Other percentiles such as the ED95 or ED99 are used when near-complete response is required.

It is compared with the median toxic dose (TD50) or median lethal dose (LD50) to calculate the therapeutic index; such comparisons are most meaningful when the curves are parallel. Being dose-based, the ED50 reflects pharmacokinetic as well as pharmacodynamic variability.

Clinical pearl

At the ED50 half of the population does not respond; clinical doses aim at a much higher proportion of responders (closer to the ED95), which leaves a narrower real margin to toxic doses than the therapeutic index suggests.
