Reversible antagonism in which the antagonist binds to the same site as the agonist, so the block can be overcome by raising the agonist concentration; the agonist curve shifts to the right in parallel without a fall in maximal response.

In depth

Agonist and antagonist compete for the same (orthosteric) site according to the law of mass action. The antagonist increases the agonist EC50 without changing Emax, reducing apparent agonist potency but not efficacy. The dose ratio (r), the factor by which the agonist concentration must be raised to restore the same response (EC50′ with antagonist / EC50 without), depends on the antagonist concentration [B] and its equilibrium dissociation constant KB (Schild equation).

Schild analysis yields pA2, the negative logarithm of the antagonist concentration giving a dose ratio of 2; for a simple competitive antagonist the Schild plot has a slope of 1 and pA2 equals pKB. Irreversible binding at the same site (e.g. phenoxybenzamine) is insurmountable and is often termed irreversible competitive antagonism.

Clinical pearl

Because competitive block is surmountable, the outcome depends on the relative concentrations of agonist and antagonist: reversing potent or long-acting opioids may require repeated naloxone doses or an infusion.
