The time required for the plasma (central compartment) concentration of a drug to fall by 50% after stopping a continuous infusion of a given duration, the 'context' being the duration of the infusion.

In depth

Unlike the elimination half-life, the context-sensitive half-time reflects both elimination and redistribution between compartments and is derived by simulation from multi-compartment models. For lipophilic drugs with large peripheral volumes, drug accumulates in tissues during long infusions and returns to plasma afterwards, so the half-time increases with infusion duration.

Fentanyl and thiopental show a marked increase with infusion length and propofol and sufentanil a smaller one, whereas remifentanil, hydrolysed by non-specific esterases, has a short context-sensitive half-time that stays almost constant regardless of infusion duration. Because recovery may need more than a 50% fall, other decrement times (e.g. 80%) are also described.

Clinical pearl

Recovery after a prolonged infusion is better predicted by the context-sensitive half-time than by the elimination half-life; because remifentanil offset is rapid even after long infusions, postoperative analgesia must be planned before it is stopped.
