The ratio of the odds of an outcome (or exposure) in one group to the odds in another group, where the odds is the probability that an event occurs divided by the probability that it does not.

Formula

OR = (a / b) / (c / d) = (a × d) / (b × c)

In depth

In a 2 × 2 table (a, b: exposed with and without the outcome; c, d: unexposed with and without it) the odds ratio is (a/b)/(c/d), the cross-product ratio ad/bc. OR = 1 indicates no association; it is the natural measure in case–control studies and logistic regression and is widely used in meta-analyses.

When the outcome is rare (roughly below 10%), the OR approximates the relative risk; when the outcome is common, the OR lies further from 1 than the RR and overstates the effect if read as a risk ratio.

Clinical pearl

An odds ratio of 2 does not mean 'twice the risk' when the outcome is common; baseline event rates should be checked before interpreting it as a relative risk.
