The average number of patients who need to be treated with an intervention for a specified period to prevent one additional adverse outcome compared with the control treatment.

NNT = 1 / ARR NNT = 100 / ARR (%)

In depth

NNT is the reciprocal of the absolute risk reduction and is rounded up to the next whole number; an ARR of 4% gives NNT = 25. The ideal NNT is 1, and lower values indicate a more effective intervention for the stated outcome.

NNT is specific to the outcome, comparator, baseline risk and duration of treatment, so values from different trials cannot be compared unless these match. It should be accompanied by a confidence interval and weighed against the number needed to harm.

Clinical pearl

An NNT is meaningful only with its outcome and time frame, e.g., 'treat 25 patients for 5 years to prevent one stroke'; patients at higher baseline risk than trial participants have a lower NNT.
